Kay Boyle, Associate
Kay has over seventeen years’ experience of working with and managing teams in the NHS. This has included working with acute, mental health, commissioners and out of hour’s community services.
 
She has extensive knowledge of governance, patient safety, investigations, inquests and system processes.
 
Kay has led and been involved in many serious incidents and complaint investigations and has led a trust legacy homicide review. This has included working with legal teams on investigations and inquests, providing support to NHS staff and affected families throughout the
process.
Kay has worked with Facere Melius on the following assignments:
 
  • Independent Investigation into multiple adult deaths
  • Patient Safety Improvement at an NHS Foundation Trust
Kay has also worked independently on:
  • Clinical Risk Manager (Acute) in an Emergency Village
  • Investigations Officer (Acute) in the Patient Safety team to clear significant backlog of Serious Incidents, completing
    Root Cause Analysis investigations across all divisions
  • Responsible for reviewing and clearing a backlog of Duty of Candour(DoC) and mentor staff for implementation of DoC, as well as mentoring the new Head of Patient Safety in their role
  • Head of Patient Safety
  • Responsible for the management of the Trust Mortality Assurance systems and processes. Providing support  of the  mortality agenda, leading on  mortality reviews and supporting others to undertake mortality reviews 
  • Responsible for management and update of databases
  • Head of Serious Incident and Inquests
  • Worked as a Serious Incident Investigator as part of the Patient Safety team to clear significant backlog of Serious Incidents, completing
    Root Cause Analysis investigations across all divisions. Preparing and collating evidence, interviewing staff, and writing chronologies as part of in-depth reports. Acted as principle advice and assistance expert for investigators new to the process of RCA
  • Working for the Regulator to review work streams within all teams and make efficiency savings through process redesign within new budget targets
