Lesley Underwood, Associate
Lesley has over 40 years’ experience within the NHS as a nurse, manager,
consultant and advocate for patient safety and quality improvement. This has included working with acute, mental health and integrated health services. Lesley has also worked to improve the physical health of people with long-term mental issues and as an infection control nurse with a keen interest in Public Health.
Her extensive knowledge includes leading in governance, patient safety – including suicide prevention, leading and managing the inquest process within organisations and undertaking and leading investigations.
As a Consultant Nurse Lesley led and undertook many serious incident
investigations including those of homicide and suicide by people who were suffering from mental illness. She has worked extensively with the Trusts’ legal teams to support families, staff and the Coroner through the inquest process and beyond.
Lesley has worked previously on:
Nurse Consultant Infection Prevention and Control Nurse (Community); which included working with Public Health and NHSI to investigate the spread of an invasive bacteria within the community.
Clinical Risk and Safety Manager (Acute) – as part of the patient safety team undertaking and supervising the outstanding Serious Incident and Complaint Investigations, using Root Cause Analysis methodology.
As Consultant Nurse for Inquests, responsible for clearing a significant backlog of evidence required by the Coroner to proceed with outstanding inquests (Integrated Trusts – including acute, community, ambulance and mental health). This information included medical and nursing reports and staff statements and Serious incident reports. Systems developed and introduced to ensure that information was available to “interested parties” in a timely manner.
Lead Infection Prevention and Control Nurse (Mental Health) – responsible for ensuring that the Trust was compliant, and the staff were confident to receive an inspection from the CQC, in addition to managing the regular infectio prevention and control agenda.
Consultant Nurse Suicide Prevention and Serious Incident Investigation (Acute and Community) – responsible for leading and undertaking Serious within the Court. Supporting the families and staff throughout the Inquest process and providing evidence Incident Investigations and Inquest Management of those cases.
Providing staff training in Root Cause Analysis and Inquest preparation. Part of the Sign Up to Safety project within the Trust, leading on suicide prevention and suicide prevention training for all staff.
