Facere Melius provided turnaround support for over 100 breached Serious Incidents, this included coaching for investigating officers to drive up the quality of the investigations. Trust had received an inadequate rating and were subsequently assessed as requires improvement.

University Teaching Hospital NHS Foundation Trust

Facere Melius provided turnaround support for Quality Governance including; design and implement new QG integrated management structure, establish new ways of working and improve the quality of assurance provided to the Trust Board. Review and implement improvements to the approach to monitoring compliance with CQC regulations and preparation for CQC compliance. Sustained Requires Improvement Rating, avoiding a predicted Inadequate rating. 

 

Risk Management System Rebuild

University Teaching Hospital NHS Foundation Trust

Facere Melius delivered a full rebuild of the Safeguard Risk Management System including associated processes which include, risk management, incident reporting and management, management of the central alerting system and patient experience. Trust was criticised in a Well Led review for lack of progress with their Risk Management System.

Facere Melius completed a full management audit and review of the Safeguard Risk Management System Implementing rapid improvements and proposing recommendations to make the system fir for purpose.

Combined NHS Trust

Providing rapid turnaround to corporate and quality governance processes. Facere Melius reviewed and segmented the full risk register and then focussing on high level risks, created a working board assurance framework that would assure CQC of improved board grip on risk and completed a review and implemented rapid improvements to the Datix Risk Management System.

 

Governance Review

Combined NHS Trust

The Trust was in the process of being placed into special measures fowling its second ‘Inadequate CQC inspection. Facere Melius provided advisory to the board on corporate and quality governance matters whilst conducting a rapid review of quality and corporate governance arrangements bringing together information from the NHSI, Capsticks Well-led review and CQC report to create a practical plan of action that would have rapid high impact changes. Our work also included an annual board review.

 

Quality Governance Review

University Teaching Hospital NHS Foundation Trust

Quality Governance review, the Trust is currently rated as ‘Requires Improvement’ and wanted an independent review of its quality governance arrangements from board to ward. The review has put forward 58 recommendations for improvement and delivered some ‘difficult messages’ to the board which were delivered in a constructive and supportive way. 

 

Quality Governance Turnaround

Hospital NHS Trust

Quality Governance turnaround in an acute trust assessed by CQC as inadequate for Safety improved to ‘requires improvement’ and taken out of special measures (6 months). Developed and delivered a transformation programme to put in the key foundations to improve the CQC rating.

 

Quality Governance Review

Working within the Clinical Services Directorate to assess the maturity of the clinical governance processes within the services provided, coaching staff and facilitating development plans to further strengthen the clinical governance and service improvement plans.

 

Hospital Trust Quality Governance Turnaround

Delivered a quality and safety turnaround project. Following a CQC improvement notice the Trust was placed in special measures. Development of new quality and safety governance structures, management groups and accountability framework for a new way of working. Coaching of top teams within divisions and directorates. Support to the executive team and non-executives on quality and safety management and assurance. Delivery of a development programme for divisions, directorates and corporate support.

 

Assurance of clinical governance and safety systems for a CSU

Working within the Clinical Services Directorate to assess the maturity of the clinical governance processes within the services provided, coaching staff and facilitating development plans to further strengthen the clinical governance and service improvement plans.

 

Capsticks Alliance 

Programme managing for the Capsticks Alliance the bidding process to support the London CCG Pathfinders and responsible for the mobilisation and delivery of a £500K support programme for Wandsworth and Hounslow CCG Pathfinders.

 

Private Medical Consortia
 

Facilitating the constituent companies to prepare for and enable registration with the healthcare regulator, skilling up the Directors to be registered managers and to provide the right competencies and assurance to the Care Quality Commission.

Combined Healthcare Trust – Quality and Safety Development

 

Completing a management review of the quality and safety structures within the orgsanisation and developing a maturity matrix for newly established directorates. Support and coaching for the leadership of the quality and safety support team.

 

Hospital Trust – Clinical Audit Workshop

 

Running a workshop on reviewing the organisations maturity to clinical audit building on the national guidance for boards and partners published by HQIP and GGI.

 

CCG – Governance review and turnaround

 

Working with a CCG in legal direction, completed a review of governance arrangements across key governance domains then making recommendations for improvement. Providing turnaround support to the CCG leading to legal directions being lifted. 

 

University Hospital Trust – Quality and Safety Turnaround

 

Programme managed and delivered key aspects of a quality and safety turnaround project. The programme was established following an CQC improvement notice and the Trust being placed in special measures. It achieved establishing new ways of working in the hospital divisions and corporate support team, delivering the foundations for sustainable improvement.

 

Welsh Hospital Trust – Business delivery structures

 

Working directly for the Chief Executive proving professional advisory service on establishing a new way of working, and undertaking a mapping exercise of good practice to plan a safe transition from the current structure to the future state.

 

University Hospital Trust – Serious Incident Recovery

 

Leading a recovery programme of a backlog of over 100 serious incidents, including completion of root cause analysis and investigation reports. Helping the Trust identify theme and trends and opportunities for improvement.

 

CCG – Investigation of Continuing Healthcare Complaint

 

Supported the delivery of an investigation report into a continuing healthcare complaint involving a commissioning support unit and a clinical commissioning group.

 

Federated CCG – Review of Governance Arrangements

 

Supported a diagnostic review of governance systems across three federated clinical commissioning groups.
