Lesley Le-Pine, Associate
Lesley has worked as a senior manager with extensive 35 years experience in the management of Patient Experience and Clinical Governance within NHS Trusts, local authority and voluntary sector settings.

Educated to Masters’ Degree level, she has significant knowledge of leading, managing, developing patient safety systems, strategies and policies whilst bringing innovation into services and involving patients.

Lesley has experience in commissioning and delivering serious incident and never event investigations in line with the national policy at all levels (1-3) and has experience of training investigating officers within the NHS environment.
Lesley has most recently been working with Trusts to deliver turnaround in breached Serious Incident Investigations and has been commissioned to complete independent investigations for an NHS Hospital Foundation Trust.

Lesley has worked with Facere Melius on the following assignments:


  • Serious Incident Tunraround of over 100 investigation at an NHS Hospital Trust

  • Serious Incident Investigator to NHS Trust  


Lesley has also worked independently on:

  • Head of Clinical Governance, Lesley was responsible for leading clinical governance and patient experience services across the Trust,
    managing complaints, clinical incidents, clinical audit, clinical effectiveness, chaplaincy, bereavement services, adult safeguarding and patient engagement

  • Providing consultancy on governance practice for both Photosymbols and Mothers for Mothers

  • Delivered RCA training for Investigating Officers throughout the Trust

  • Interim Quality and Patient Engagement Lead – reviewing providers serious incidents, investigating GP complaints about providers, undertook review of suicide serious incident
