Patient Safety Incident Response Framework
​The Patient Safety Incident Response Framework (PSIRF) sets out the NHS’s approach to developing and maintaining effective systems and processes for responding to patient safety incidents for the purpose of learning and improving patient safety.

Facere Melius are the only approved training supplier that worked closely with NHS England in developing tools and guidance to support PSIRF.
We have developed training courses to support providers to prepare, transition and work under the new Patient Safety Incident Response Framework.
​
Our courses provide a safe space to discuss and debate the challenges of implementing the PSIRF framework as well as providing delegates with example tools, techniques and policies that can be used and adopted locally.
OUR PSIRF TRAINING COURSES
PSIRF Standards our courses meet
Learning responses are led by those with at least two days’ formal training and skills development in learning from patient safety incidents and experience of patient safety incident response.
Engagement and involvement with those affected is led by those with at least six hours of training in involving those affected by patient safety incidents in the learning process.
All patient safety incident response oversight is led/conducted by those with at least two days’ formal training and skills development in learning from patient safety incidents and one day training in oversight of learning from patient safety incidents.

Meet the Facilitators
Howard Scott
Howard has expertise carrying out governance reviews in healthcare organisations and working with NHS Boards to improve their safety culture. Originally trained as a mental health nurse has a significant experience in patient safety and has delivered all of the courses in our PSIRF development programme.
Darren Thorne
Darren is a recognised leader in Quality, Safety and Governance, he has chaired national networks in Clinical Effectiveness, Audit and Governance and has previously provided professional advisory services to national bodies including the healthcare regulator, national patient safety agencies and the national clinical audit programme.
